We use a student/parent/teacher survey every year that is taken online. We use this information to help with perception of curriculum and other activities throughout the year.
I was very interested in the perception answers from my students as I push to implement "flipping" the classroom next year. I used this survey with my current class, but plan to use it (with some additions) to the fourth graders before they leave for the summer to get an idea of their needs.
